Parent Intake Form
Approximately one week prior to your parent interview, please return the Parent Intake Form. This collects some important history about your child and will be used as part of their evaluation. This form should have been sent to the parent through the secure patient portal too.
Parents are encouraged to return the completed PDF fillable form through the portal, or can also email the form directly to Dr. Katie.
