Here is some additional information regarding insurance, superbills, CPT codes, and when payment is due.

The specific fees for each evaluation are outlined by Type of Evaluation

Insurance

I do not take insurance, but at the conclusion of the evaluation, I am happy to provide you with a superbill. Please know that I will be an out-of-network provider. I recommend that you check with your insurance company prior to the evaluation to determine what you might be reimbursed.

Dr. Katie will email you the information provided by Reimbursify.

Once we have partnered together and the evaluation is completed, Dr. Katie will assist you in getting started on the process with Reibursify by providing a Superbill. You will then work with Reimbursify from that point on the reimbursement process.

What is a superbill

A superbill is a document used to request reimbursement insurance companies for services from medical providers. It is typically provided to patients after they have received some type of medical treatment and includes detailed information about the services they were provided. The purpose of a superbill is to provide the insurance companies with the information they need to process a claim for reimbursement.

What will be included in your superbill:

  • The dates of service.

  • My name (provider) and information (e.g., address, NPI number, license number).

  • Your child’s name and information (e.g., address).

  • Diagnoses codes (these are the codes that represent what diagnosis if any your child has been given. If there is no diagnosis the insurance company likely will not pay for the evaluation).

  • Procedure codes (also known as CPT codes).

  • The total cost of service (I will note that you have already paid in full for the evaluation).

CPT Codes

Here is a list of the CPT Codes that I will use. You may reference these if you speak with your insurance. 

  • 96116 – This is used to code for the parent interview.

  • 96136 / 96137 – These code are used to capture the time spent testing and scoring the tests.

  • 96132 / 96133 – These codes are used to capture the time spent reviewing records, conceptualizing your child’s case, writing the report, and providing feedback.

When is Payment Due

Payment is due at the first in-person testing session. At this time, you can pay by check, money order, cash, or some credit cards (visa and master card).
